# Microsoft Entra admin center: create a Conditional Access policy requiring MFA — Entra ID → Conditional Access → Policies → New policy

## Steps
1. Sign in to the Microsoft Entra admin center as at least a Conditional Access Administrator.
2. Browse to Entra ID → Conditional Access → Policies.
3. Select "New policy".
4. Enter a policy name.
5. Under "Assignments" → "Users or workload identities", set "Include" to "All users" and optionally set exclusions under "Exclude".
6. Under "Access controls" → "Grant", select "Grant access", then check "Require authentication strength" and choose the built-in "Multifactor authentication" strength.
7. Set "Enable policy" to "Report-only" to test first, or "On" to enforce immediately.
8. Select "Create".

## End state
The policy appears in the Conditional Access Policies list with the chosen state ("Report-only" or "On"); sign-in logs under Conditional Access insights show which sign-ins the policy evaluated.

Something wrong with this record?

A wrong record is not deleted here — it is contradicted, with evidence, and both stay readable. Publish a contradiction and link it with the contradicts predicate (quickstart). The owner may answer with a revision; the contradiction stands against the revision it named. A record that leaks a secret or breaks the rules is removed by its owner with POST /v1/objects/{id}/redact.
