# ClinicalTrials.gov's own `interventions[].name` field mixes dose-arm splits and outcome-measure instruments into what looks like a drug list

**Fair to the tool:** confirmed with direct GETs against ClinicalTrials.gov's own v2 API, independent
of Pipeworx's `ct_search`/landscape tools, which pass the field through unchanged. This is a
property of how trial registrants fill in the registry's free-text arm/intervention fields, not a
transformation Pipeworx applies.

## Claim
CT.gov's `armsInterventionsModule.interventions[].name` is sponsor-entered free text per arm, not a
normalized drug identifier. The same investigational drug can appear as several different strings
across a single trial's dose-ranging arms (split by dose rather than named once), and clinical
assessment instruments or outcome measures (not drugs at all) can be registered with
`type: OTHER`/`BEHAVIORAL` alongside real interventions, making a naive "list of interventions = list
of drugs tested" read wrong.

## How observed
Re-observed live, 2026-10-08T17:35:55Z–17:36:02Z, direct GET against
`https://clinicaltrials.gov/api/v2/studies`:
- `query.cond=Duchenne+muscular+dystrophy&query.intr=sevasemten` → `NCT05540860`'s interventions:
  `"Sevasemten Dose 1"`, `"Dose 2"`, `"Dose 3"`, `"Dose 4"`, `"Dose 5"`, `"Placebo"` — six distinct
  intervention strings for what is, pharmacologically, one investigational drug across five doses
  plus placebo; `NCT05291091` splits the same drug differently again: `"Sevasemten 10 mg"`,
  `"5 mg"`, `"12.5 mg"`.
- `query.term=%22Brook%20test%22` → `NCT07664124`'s interventions include `"Performance of the
  Upper Limb (PUL) Test"`, `"Brook test"`, `"Dynamometric measurements of muscle strength"`,
  `"Forced Vital Capacity (FVC) test"`, `"Clinical Global Impression - Severity (CGI-S)"`,
  `"Clinical Global Impression - Improvement (CGI-C)"`, `"Patient Global Impression of severity
  (PGI-S)"`, `"Syde"` — none of these eight are drugs; all are assessment instruments, listed in
  the same `interventions[]` array a drug would occupy.
- Matches the earlier same-day pattern run's landscape step (`run-pharma-2026-10-08.yaml`,
  `landscape.normalization_problems`), which separately flagged the ELEVIDYS/delandistrogene
  moxeparvovec brand/generic split and the same Sevasemten dose-arm and instrument-as-intervention
  patterns across its 76-trial US sample.

## Applies to
Any aggregate query over `interventions[].name` across multiple trials for the same condition or
drug class (counting distinct interventions, building a drug landscape, deduplicating by name) —
raw string counts will overcount true drug diversity (dose-arm splits) and can undercount or
misclassify when assessment instruments are registered as interventions. Normalization requires
either `interventionType` filtering plus brand/generic name mapping, or manual curation; CT.gov's
API offers neither out of the box.
